One-to-one Therapy - Confidentiality and access to case notes

The trust between client and therapist is crucial to the success of the therapeutic process and I treat all information disclosed as confidential. Psychologists are required to discuss their work regularly with a supervisor, - a qualified psychologist or specialist in the appropriate field. Any details my supervisor receives are also treated as confidential and we do not disclose client details to any third party without the client’s permission.  However, if in my opinion, a client is either a danger to themselves or to others, or there is a possibility a child might be at risk, I do reserve the right to inform appropriate agencies.  It is my practice wherever possible to inform the client first should confidentiality need to be breached.  I keep brief notes on our work together session by session,  which you are entitled to see at any time if you wish. These will be deleted after a year. They are kept for reference in case you decide to return to therapy within that time.  They are kept on a computer cloud file that is password protected. I keep a paper summary of our work, together with any letters, securely locked for up to seven years, after which it is destroyed.

Mindfulness Courses

To attend a Mindfulness course run by me, you will be asked to complete a form with your personal details including your name, address, date of birth, next of kin.  You will also be asked to disclose any relevant medical details, and any history of psychological difficulties.  This is essential to ensure that the course is appropriate for you, and, at worst, will not be harmful to you.  This may be via a paper form, which can be submitted by email.

Individual forms are kept together in a locked cabinet, but brought to each mindfulness session in case information is needed during the session.

Forms will be destroyed by 3 months after the end of the course.   In addition, you will be invited to sign up for my Mailchimp monthly newsletter.  This is entirely optional and you can unsubscribe at any time.

Any personal material shared during the course is entirely at your discretion.  However this  may be shared only with my supervisor, for the purpose of my own learning and reflection.  Real names will not be used.  Other course participants are asked to keep confidential anything shared within the Mindfulness sessions.
